When visiting this website, for those practicing in Texas click on the link for Texas located under the “Explore” portion of the home page.  Here you can find Newsletters, staff training, and under LCD (local coverage determination) you can receive fee schedules and search CPT codes for invaluable information.

 

Supporting Medical Necessity

 

For help in determining the level of medical necessity required to support a particular CPT code you can view the LCD portion of the Medicare website for approved ICD9 codes, global coverage period and other pertinent information.   Follow these steps;

  1. Log on to www.trailblazerhealth.com

  2. Under the "Explore" portion of the website on the left of your screen, click on the link for Texas.

  3. Read the disclaimer and then click accept to proceed

  4. Scroll down the page under the "Explore" portion will be a link for LCD (local coverage determination)

  5. Now you can search by CPT code or by description for detailed information on each CPT code including ICD9 codes that support medical necessity, fee schedule, global period and other information.

From this website you can receive information on standards for billing medical claims.  For “clean claims” filed with insurance companies that remain unpaid you can submit a complaint using their on-line complaint form by using the following link.  Your unpaid claim will then be investigated by the TDI at no charge to you.
